Referral Information

What Programs We Provide

  • Private, furnished apartment-style housing
  • Up to 24 months of stable residence
  • Case management and individualized goal planning
  • Life-skills and financial literacy support
  • Education and employment readiness support
  • Parenting and family stability support
  • Trauma-informed, dignity-centered care

Who We Serve

  • Young mothers ages 18–25
  • Pregnant or parenting a child age 5 or younger
  • Experiencing homelessness or housing instability
  • Ready to participate in a structured, goal-focused program

Referral Process

How To Refer A Client

1. Confirm immediate safety: If the client is in crisis, connect them to emergency resources first.
2. Review eligibility: Confirm age range, parenting status, and readiness for long-term transitional housing.
3. Submit a referral inquiry: Use the form below or contact our intake team.
4. Client completes application: The client will be invited to complete our formal application when appropriate.
